Intramural Sports Forms

Fill out, print, sign, and return the completed form to the Student Life Office before registration deadlines.

Waiver forms and Contact Information forms must be submitted.  Students only need to fill out one waiver form for the year, but individual Contact Information forms must be completed for each sport.
