Apply for your U.S. Passport - Everyone is eligible (on or off campus).

No passport services available on Monday
Tuesday through Friday
8:30 am to 11:00 am, by appointment only. Call 455-0570
11:00 am to 4:00 pm, walk-in services
What you need to bring:
- Passport Application (Form DS-11, found on the internet at travel.state.gov or pick one up in our office (BE-109)
- For minors, both parents need to be present. If one parent cannot be present, form DS-3053 needs to be filled out by the non-accompanying parent and then notarized.
- Two passport photos (Photos can be taken at Costco, Sam's Club, Kinko's, Wal-Mart, and Longs
- Proof of citizenship
- Previously issued, undamaged U.S. Passport
- If born in the U.S. - Original certified U.S. birth certificate, embossed, and file date no later than one year of birth date.
- If born outside of the U.S. - Original Certificate of Naturalization
- An identification card
- Valid driver's license
- Valid Military ID card
- Naturalization Certificate
