Documentation Guidelines


Students requesting accommodations should contact the KI Program at (808) 455-0421 and must provide verifying documentation of their disability. Documentation consists of an evaluation by an appropriate professional that relates the current impact of the condition to the requester.  A high school student's Individualized Education Plan or "IEP" is not sufficient for documentation purposes.

Documentation should include:

• A diagnostic statement including date of most recent evaluation (no more than two years old)
  
   
• The current impact of (or limitations imposed by) the condition
  
   
• Treatments, medications, devices or services currently prescribed or used to minimize the impact of the condition
  
   
• The expected duration, stability, or progressing of the condition
